This position serves as a bridge between clinical documentation, interdisciplinary communication, and reimbursement accuracy, supporting our commitment to delivering exceptional quality of care. Key Responsibilities Coordinate and complete MDS assessments in compliance with CMS, state, and facility guidelines. Review resident charts, nursing documentation, and therapy notes for assessment accuracy. Lead and facilitate Care Plan meetings with the interdisciplinary team to ensure resident needs and goals are met. Manage PDPM classifications and ensure correct coding for reimbursement optimization. Maintain compliance with all assessment schedules and submission deadlines. Educate nursing and therapy staff on documentation practices that impact MDS accuracy and reimbursement. Participate in QAPI, survey readiness, and performance improvement initiatives. Monitor and initiate MDS assessments for significant changes in resident condition. Qualifications Current California RN or LVN license in good standing. At least 1 year of experience completing MDS assessments in a Skilled Nursing Facility. Strong knowledge of RAI process, PDPM, and care plan development. Familiarity with PointClickCare (PCC), SimpleLTC, or related clinical documentation software. Excellent organizational, analytical, and interpersonal skills. Ability to collaborate effectively with the nursing team, therapy, and administrative leadership. Preferred Attributes Previous leadership or training experience in SNF clinical documentation.
