Job Overview We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will have experience in various settings, including hospitals and memory care facilities. This role requires a strong focus on patient care, pain management, and discharge planning. The Registered Nurse will be responsible for providing high-quality nursing care to patients, ensuring their comfort and safety throughout their treatment. Duties Provide comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed, ensuring adherence to aseptic techniques. Monitor patients' vital signs and overall health status, intervening as necessary to address any concerns. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure continuity of care. Utilize eClinicalWorks for documentation and communication regarding patient care. Conduct MDS assessments for appropriate patient classification and care planning. Educate patients and their families on health management, pain management strategies, and post-discharge instructions. Assist in ventilating patients when necessary, ensuring proper techniques are followed. Qualifications Current Registered Nurse (RN) license in the state of practice. Experience in inpatient settings preferred; familiarity with memory care is a plus. Proficient in pain management practices and discharge planning processes. Strong understanding of MDS documentation and eClinicalWorks software is desirable. Excellent communication skills and the ability to work collaboratively within a healthcare team.
