Full Job Description Position Summary:
We are seeking a detail-oriented and knowledgeable MDS Coordinator to join our team. The MDS Coordinator is responsible for managing the Minimum Data Set (MDS) process, ensuring timely and accurate assessments, and supporting compliance with federal and state regulations. This role plays a key part in resident care planning and reimbursement processes.
Key Responsibilities:
Complete and coordinate all MDS assessments in accordance with state and federal guidelines. Ensure accuracy and timeliness of MDS submissions. Collaborate with interdisciplinary team members to develop, implement, and monitor care plans. Monitor regulatory changes and update processes to maintain compliance. Educate and support staff regarding MDS requirements and documentation standards. Maintain accurate resident records and ensure confidentiality.
Qualifications:
Current RN or LVN/LPN license. Previous experience with MDS 3.0 and care planning in a long-term care setting. Strong knowledge of federal and state regulations related to MDS and reimbursement. Excellent communication, organizational, and analytical skills. Proficiency with electronic health records (EHR) and Point Click Care.
