MDS. MUST HAVE MDS COORDINATOR EXPERIENCE
Under the direction of the DON, the MDS/RAC is responsible for conducting and coordinating the completion of the resident comprehensive assessment (RAC) which includes the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and the RAP summary sheet. The coordination of periodical comprehensive, interdisciplinary assessments upon admissions, annually and whenever there is significant change of condition. JOB REQUIREMENTS and
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or abilities required. Graduate from accredited school of nursing; Current licensure as a Registered Nurse or LVN in the state in which practicing; evidence of training in resident assessment. Strong communication skills, oral and written. Basic mathematical skills required. Demonstrates ability to understand and follow State and Federal regulations and established policies and procedures of the facility; plans, organizes and prioritizes work assignments efficiently. Functions efficiently and effectively with minimum direction. Able to coordinate different levels of health care professionals to assess the needs of residents based on a minimum data set and develop a comprehensive plan of care. Self-starter, detail oriented and strong interpersonal skills.
