Responsibilities include completing patient admissions, developing individualized care plans, coordinating interdisciplinary services, and supporting positive patient outcomes in the home setting. Key Responsibilities Obtain medical histories and assess patient conditions and clinical needs Conduct comprehensive nursing assessments including vital signs and mobility evaluations Educate patients and families on treatment plans, disease management, and home health services Deliver skilled nursing care while maintaining HIPAA compliance Complete accurate and timely clinical documentation and visit records Develop, implement, and revise individualized care plans as appropriate Coordinate care with LVNs/LPNs, Home Health Aides, therapists, and additional interdisciplinary team members Facilitate referrals for additional services including rehabilitation and medical social work Evaluate patient response to treatment and adjust care plans accordingly Manage assigned caseload while meeting 30-point weekly productivity expectations Qualifications Active California Registered Nurse (RN) license Home Health experience preferred Experience with OASIS and WellSky (Kinnser) strongly preferred Med-Surg and wound care experience preferred Current physical examination completed within the last 6 months Current TB test (within the last 12 months) Current CPR certification Valid driver's license and reliable transportation Must be able to attend orientation at our Yuba City office Where We Serve Aspire offers opportunities across AZ, CA, CT, FL, IL, IN, MA, MN, NV, and OH .
