Rancho Cucamonga, CA Job Details Part-time $45 - $52 an hour 1 day ago Qualifications Terminal illness counseling Teamwork RN License Case management End-of-life care Time management Full Job Description RN part-time position for the Palliative Care Program. Responsibility includes admission and assessment of qualified patient to the palliative care program. The admission RN is responsible for comprehensive assessment, developing a care plan and collaborating with the Palliative team. The RN must have a minimum of 3 years' experience in home health, hospice or palliative work experience. In addition, the qualified RN must also possess a minimum of 2 years' clinical experience in inpatient and/or critical care settings.
Core Competencies:
Knowledge in pain control, symptom management, chronic conditions, and end of life in adult and geriatric population.
Communication:
strong communication for sensitive family discussions, high level of empathy and emotional resilience, ability to coordinate care effectively across different disciplines.
Other qualities:
self-directed, efficient and effective time management, strong documentation skills, flexibility, ability to supervise other disciplines including LVN, aide, chaplain and social worker, demonstrates effective work collaboration included but not limited to case managers, physicians, pharmacy and UM department.
Work hours:
8:30-5:00 pm Part-time requirement: minimum of 3 days per week No weekend work requires Holiday work rotation Efficiency with technology and EMR documentation system Strong word and excel program abilities May cross over assignment in the hospice program, as needed.
Hourly Rate:
$45-$52
Pay:
$45.00 - $52.00 per hour
