Position:

The MDS Coordinator assumes full time administrative authority, responsibility and accountability to coordinate the completion of the required minimum data set and with other members of the inter-disciplinary team, develops and implements a plan of care that meets the individual needs of each resident.

Job Functions:

Provide quality, resident-centered care while protecting resident rights and confidentiality. Follow facility policies, HIPAA, and all applicable regulations. Maintain knowledge of long-term care practices, admissions, reimbursement, and MDS/RAI processes. Communicate effectively with residents, families, staff, and regulatory agencies. Follow safety, infection control, and emergency procedures. Prioritize tasks, maintain professionalism, and complete assigned duties. Perform other duties as assigned.

QUALIFICATIONS/REQUIREMENTS

Education:

Graduate of an accredited school of nursing

License:

Current active license as a Registered Nurse or Licensed Vocational Nurse (LVN)

Work Experience:

At least 6 months to 1 year of MDS experience SNF experience preferred
