This vital role combines clinical excellence with personalized patient engagement, ensuring individuals receive safe, effective treatment tailored to their unique needs. As a key member of our home health services, you'll coordinate care plans, perform assessments, and deliver essential nursing interventions that promote healing and well-being. Your energetic approach and dedication will help foster a positive environment for both patients and colleagues alike. Duties Conduct thorough patient assessments including vital signs, physical examinations, and functional evaluations to develop individualized care plans. Administer medications via IV infusion, injections, and other routes while adhering to aseptic techniques and safety protocols. Manage complex clinical procedures such as catheterization, wound suturing, ventilator management, and ostomy care with precision and confidence. Provide education on disease management, medication adherence, nutrition, and self-care strategies to patients and their families. Collaborate with interdisciplinary teams using electronic health records (EHR/EHR systems) like Epic or Cerner to coordinate care effectively. Monitor patient progress through diagnostic evaluation and laboratory testing; adjust treatments accordingly to optimize outcomes. Ensure compliance with HIPAA regulations and maintain meticulous medical documentation for legal and quality assurance purposes. Requirements Valid RN license in the state of practice with current CPR/BLS certification. Critical care experience in settings such as ICU or trauma centers is highly preferred. Demonstrated proficiency in EMR/EHR systems like Epic or Cerner; experience with medical coding (ICD-10/ICD-9) is a plus. Strong knowledge of physiology, anatomy, pharmacology, and infection control practices. Experience working with diverse patient populations including pediatrics, geriatrics, individuals with developmental disabilities or dementia care needs. Skilled in performing complex procedures such as IV insertion, phlebotomy, tube feeding management, and telemetry monitoring. Excellent communication skills with the ability to triage urgent situations effectively; familiarity with Workers' Compensation law and hospice & palliative medicine is advantageous.
