POSITION SUMMARY

The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes under the supervision of a physician. The RN is responsible for implementing start-of-care plans, administering treatments and medications, performing wound care, coordinating discharges and collaborating with the clinical team to achieve positive patient outcomes.

ESSENTIAL FUNCTIONS

Perform case management and follow-up skilled nursing visits in the patient's home. Conduct patient assessments, consult with physicians, and develop individualized treatment plans. Submit regular progress reports to the attending physician and clinical team. Coordinate and complete patient discharges in consultation with the physician and clinical manager. Deliver direct nursing care and prepare written care plans. Educate and support patients and families to ensure care plan compliance. Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA) Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient's condition and needs. Manage and coordinate the plan of care in a professional manner. Complete timely, accurate, and compliant clinical documentation.

REQUIRED EXPERIENCE
