The ideal candidate will possess a strong commitment to serving a geriatric population, patient care and safety, with the ability to work in various clinical settings. This role involves providing high-quality nursing care in a Primary Care setting as well as periodic home visits and collaborating with our Interdisciplinary Team (IDT). Duties Assess and monitor patient health status, including vital signs and medical history. Develop and implement individualized care plans based on patient needs. Administer medications and treatments as prescribed by physicians. Provide education and support to patients and their families regarding health management and treatment options. Collaborate with interdisciplinary teams to coordinate patient care effectively. Maintain accurate medical documentation in compliance with healthcare regulations. Participate in emergency response situations, utilizing trauma medicine and emergency medicine skills as necessary. Engage in home care visits as needed. Skills Proficient in medical documentation practices, ensuring accurate record-keeping. Familiarity with DME (Durable Medical Equipment) usage and management. Experience working geriatric populations. Strong communication skills for effective interaction with patients, families, and healthcare teams. Ability to remain calm under pressure while providing compassionate care.
