Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Join our growing team! CareOne Home Health is locally owned and growing company seeking an RN Case Manager to cover Stanislaus County and surrounding communities. Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. Home health experience ideal, and we will provide training and support. This is a full-time position with rotating on call requirements. Benefits Competitive salary Focused one to one patient care Medical, dental and vision insurance Flexible work schedule & reasonable work hours 401k matching Supportive and friendly work environment Requirements Minimum of 1 year of experience as a Registered Nurse with a valid license required Current CPR/BLS Certification Reliable transportation required Experience with Kinnser strongly preferred Comfortable driving to patient homes for patient visits Prior experience with Palliative Care preferred Prior experience in Home Health, Hospice, Infusion, or Home Care preferred Responsibilities Initiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient, physician, and other team members when appropriate. Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services. Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient's home or living facility. Communicate necessary revisions to patient status and/or needs with the entire care team Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning. Provide complete, accurate and timely documentation consistent with the plan of care
