Job Description

Join Care Unlimited Health Services! Immediate Visits Available! We are immediately hiring compassionate and skilled Licensed Vocational Nurses (LVNs) to provide high-quality, one-on-one care in the comfort of our patients' homes throughout the San Gabriel area (Alhambra, San Gabriel, San Dimas, Glendora and West Covina). All positions are open to start immediately following our successful onboarding process.

Full-Time or Part-Time options
Flexibility:
Weekday and Weekend visits available.
Compensation:
Competitive Pay Per Visit ($40 to $45 Hr.) LVN Field Job Description As a vital member of our field team, you will provide skilled, personalized care across your assigned San Gabriel territory:
Medication Management:
Prepare, administer, and accurately record medications per MD orders and protocols during home visits.
Clinical Compliance:
Perform all duties in compliance with HIPAA guidelines, agency standards, and electronic charting procedures.
Patient Advocacy:
Act as the eyes and ears in the home, reporting directly to the Case Manager, RN Supervisor, and Director of Nursing (DON).
Travel Requirement:
Daily travel to patient homes within the San Gabriel area is required. Qualifications & Requirements We are seeking a dedicated LVN with the following:
Must-Haves:
Licensure:
Current California State LVN Licensure.
Certifications:
Current CPR/BLS Certificate.
Health:
Current TB/Physical Clearance and Negative TB Test Result.
Transportation:
Valid CA Driver's License and reliable auto insurance.
Preferred Skills:
Experience with Electronic Home Health Documentation (e.g., Kinnser) Bilingual (Spanish, Mandarin, or Cantonese) is a plus!
