In this role, you will bring high-quality, relationship-based care directly to patients through home visits, community-based care, and virtual appointments while working within a patient-centered, value-based care organization focused on improving outcomes and supporting long-term health. Our care model is focused on quality outcomes, preventive care, proactive interventions, and coordinated patient support rather than high visit volume. You will enjoy a high level of clinical autonomy while working alongside an interdisciplinary team dedicated to helping patients achieve better health outcomes.

Monday-Friday | 8:00 AM-5:00
PM Competitive Base Salary:
$130,159-$195,238 $10,000
Sign-On Bonus Mobile Practice Setting:
Home visits, community-based care, and virtual appointments
Mileage Reimbursement Quarterly Incentive Plan:

CME Support:
5 days of CME leave plus $2,000 reimbursement Autonomous practice environment with strong interdisciplinary support Value-Based Care Model focused on proactive interventions, preventive care, quality outcomes, and coordinated services Qualifications Active and unrestricted California Nurse Practitioner license Active DEA license Current BLS certification Strong clinical assessment and diagnostic skills Ability to practice independently while collaborating effectively with an interdisciplinary care team Commitment to patient-centered, preventive, and value-based care Excellent communication and relationship-building skills Comfortable providing care in home, community, and virtual settings
