This vital role offers the opportunity to work independently while collaborating with multidisciplinary teams to promote health, prevent illness, and support recovery. As a key member of our home health services, you will utilize your clinical skills and passion for patient-centered care to foster independence and improve quality of life for diverse populations across various settings. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans. Administer medications, including injections and IV infusions, following strict aseptic techniques and medication protocols. Manage complex wound care procedures such as suturing, ostomy care, and dressing changes while ensuring infection control standards are maintained. Provide specialized treatments including Botulinum toxin therapy and pain management strategies tailored to individual needs. Educate patients and their families on disease management, medication administration, nutrition, and self-care techniques to promote optimal health outcomes. Coordinate with physicians and interdisciplinary teams utilizing EMR (Electronic Medical Records) systems like Epic or Cerner for accurate documentation and seamless communication. Perform diagnostic evaluations such as laboratory specimen collection, processing, and interpreting results to inform ongoing treatment plans. Assist with discharge planning, case management, and triage services to ensure smooth transitions between care settings. Work with individuals with developmental disabilities, memory care needs, or those requiring hospice & palliative medicine support to deliver compassionate, specialized care. Qualifications Valid Registered Nurse (RN) license in good standing with current state requirements. Proven experience in home health nursing or related fields such as medical-surgical, critical care, ICU, or hospital medicine. Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth for accurate medical documentation. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques. Experience with IV insertion, catheterization (including Foley and other urinary catheters), tube feeding management, and aseptic technique. Ability to perform physical examinations and assessments in diverse patient populations including pediatrics (neonatal ventilator) and geriatrics (memory care). Excellent communication skills for patient education, family coaching, and interdisciplinary collaboration. Critical care experience such as ICU or Level I Trauma Center exposure is highly preferred. Knowledge of infection control practices and HIPAA regulations to ensure patient privacy and safety.
