A leading home health organization in San Marcos, California is seeking an RN Case Manager to join its growing clinical team!
Pay:
$55.00 - $65.00/hourRN Case Manager Opportunity:
Full-time, permanent, direct-hire position Monday-Friday, 8: 00 AM - 5: 00 PM schedule Weekend and after-hours on-call rotations $12, 000 sign-on bonus available Comprehensive benefits package including medical, dental, vision, retirement benefits, and paid time offRN Case Manager Responsibilities:
Complete patient assessments and determine home care needs Develop and revise individualized plans of care Administer medications and treatments as ordered by physicians Educate patients and families regarding treatment plans and healthcare needs Coordinate care with physicians and interdisciplinary healthcare teamsRN Case Manager Qualifications:
Current California RN license required Minimum of one year of recent acute care experience required Current BLS/CPR certification required Valid driver's license and auto insurance required Home health experience preferredThe Surrounding California Community:
