A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families. We are currently seeking a Registered Nurse to serve as a Hospice Nurse Case Manager . This person will be responsible for the provision of skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

Monday•Friday; 8:30am•5:00pm
Location:

Essential Duties & Responsibilities include (but are not limited to): Provide care in the home and work as a member of the interdisciplinary team comprised of nurses, social workers, chaplains, hospice aides and physicians. Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care.
Education/Experience :
One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing. Experience in acute care/ home health and/or hospice setting preferred but not required.
Certificates, Licenses, Registrations:
Current license as a registered professional nurse in the State of California. Public Health Nursing certification highly desirable. HPNA certification preferred. Current California Driver's License and automobile insurance that meets State of California requirements, if driving a car.
Pay Range:
