POSITION SUMMARY
Must hold valid Oregon nursing license The Regional Nurse Home Care Case Manager oversees clinical standards, ensures compliant care delivery, and trains Caregivers across our in-home care operations. This role is essential to maintaining safe, high-quality care for our Clients while supporting regulatory compliance and continuous clinical improvement.

ROLE RESPONSIBILITIES
  • Oversee nursing services and clinical standards, including the medication administration program, ensuring compliance and consistency
  • Conduct Client nursing and medication evaluations, identifying risks and ensuring safe, high-quality care delivery
  • Train and verify Caregiver competency in medication administration, policies, and nurse-delegated tasks
  • Maintain accurate and compliant clinical documentation aligned with company protocols and state regulations
  • Review physician orders, reports, and care plans for accuracy, completeness, and regulatory compliance
  • Collaborate with community providers and internal teams to coordinate care and support positive Client outcomes
  • Ensure adherence to all in-home care licensing standards, regulatory requirements, and company policies
  • Support continuous improvement of clinical practices, training, and care quality across the organization
ROLE QUALIFICATIONS
  • Active RN license in good standing; clinical experience in home health, in-home care, or a regulated care setting required
  • Strong knowledge of medication administration protocols, nurse delegation, and MAR documentation
  • Familiar with in-home care licensing standards, state/federal regulations, and HIPAA compliance requirements
  • Excellent clinical judgment with experience conducting Client assessments and evaluations
  • Detail-oriented with accurate, compliant documentation and recordkeeping practices
  • Strong communicator and collaborator with the ability to train, support, and evaluate Caregiver staff
  • Proactive and solutions-oriented with the ability to manage multiple priorities across locations
  • Experience in multi-site clinical oversight or home care case management preferred
