RN Case Manager Hospice

Serenity Hospice is looking for a Registered Nurse Case Manager to join our team and provide home health, hospice and palliative medicine services to our patients. The ideal candidate will have experience in providing compassionate care to patients and their families, and will be able to effectively manage a caseload of patients. The successful candidate will also be responsible for coordinating with other healthcare professionals, maintaining accurate records, refilling/ordering medications, supplies and DMEs and providing education and support to patients and their families.

Responsibilities:

Provide hospice services to patients in accordance with physician orders and visiting nursing report Order and refill medications, DMEs, supplies as needed for patients assigned Audit and ensure quality of patient medical record charting, including nursing notes, orders, and care plans. Assess patient needs and develop individualized care plans. Monitor patient progress and update care plans as needed. Provide education to patients and families on disease processes, medications, and self-care techniques. Coordinate services with other health care providers such as wound specialists, PT comopanies, visiting nurses, as well as Doctors. Document patient care activities in accordance with regulatory requirements.
