Job Description

Horizons Care Center is seeking a compassionate RN to provide skilled nursing care to residents in our long term care and short term rehab units. The RN will complete assessments, administer medications and treatments, monitor changes in condition, and coordinate with physicians, therapists, and families to support individualized care plans. Responsibilities include supervising CNAs, maintaining accurate documentation, promoting safety and infection control, and participating in quality improvement. The ideal candidate is patient focused, a strong communicator, and thrives in a collaborative, interdisciplinary team environment. Responsibilities Provide direct nursing care to residents in long term care and rehab units Complete ongoing assessments and monitor changes in resident condition Administer medications, treatments, and wound care per provider orders Develop and implement individualized care plans with the care team Supervise and support CNAs and other support staff Maintain accurate and timely clinical documentation Communicate effectively with residents, families, and providers Promote resident safety, infection control, and quality standards Participate in interdisciplinary care conferences and rounds Support quality improvement and regulatory compliance activities Required Skills Patient assessment Medication administration Care planning Wound care IV therapy Electronic medical records (EMR) Infection control Clinical documentation Supervision of CNAs Care coordination
