- Home Health (Montezuma & La Plata Counties) Guardian Angel Home Health is hiring a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide skilled home health visits throughout Montezuma County and La Plata County . We serve patients in Cortez, Durango, Bayfield, and surrounding communities .This posting is listed under Cortez for Indeed location purposes , but we are actively hiring nurses for both counties and match clinicians with local clients whenever possible . What You'll Do Provide skilled nursing care in patients' homes Monitor patient conditions and report changes as needed Administer medications and treatments per physician orders Perform routine nursing tasks (vital signs, wound care, injections, etc.) Educate patients and families on care and healthy habits Complete accurate and timely documentation Follow all safety, infection control, and agency policies What We're Looking For Current RN or LPN license (Colorado) Ability to work independently and manage visits efficiently Strong communication and patient-care skills Compassionate, dependable, and professional approach Home health experience preferred, but not required Pay & Schedule Per-visit pay , based on license, experience, and in person interview Full-time and Part-time positions available Flexible scheduling About Guardian Angel Home Health Locally operated home health agency serving Montezuma and La Plata Counties Focused on one-on-one patient care in the home setting Supportive team environment with realistic expectations We value professionalism, communication, and work-life balance Guardian Angel Home Health is an equal opportunity employer and encourages all qualified individuals to apply.
