An opportunity is available for an experienced and compassionate Home Health Registered Nurse to provide skilled care to patients in their homes. This contract position centers around delivering high-quality, patient-centered nursing services, focusing on assessments, individualized plans, and effective communication with both patients and interdisciplinary teams.

Schedule:

Monday Friday, 8:00 AM 5:00 PM 13-week contract

Caseload:

2532 points per week (typically 35 patient visits per day) Field-based with daily travel to assigned patient homes (mileage reimbursement provided)

Responsibilities:

Complete comprehensive OASIS assessments and develop individualized care plans. Deliver a wide range of skilled nursing tasks including wound care, IV therapy, chronic disease management, and medication administration. Evaluate patient condition, report changes, and coordinate seamlessly with healthcare professionals. Educate patients and families on disease processes, medications, and best practices in self-care. Maintain detailed, timely documentation using Home Care Home Base (HCHB). Collaborate with therapists, social workers, and aides to ensure care is integrated. Participate in care conferences, meetings, and professional development, adhering to regulatory and agency guidelines.

Requirements:

Active RN license in good standing for the state of practice. At least 1 year of recent, hands-on home health nursing experience. Working knowledge and experience with Home Care Home Base (HCHB) and OASIS assessments. Strong clinical assessment, critical thinking, and communication abilities. Ability to work autonomously and efficiently manage a field schedule. Reliable transportation, a valid drivers license, and insurance. BSN and prior experience with 2532 points/week preferred. Familiarity with Medicare and home health compliance standards is a plus.
