Monday through Friday, 8:00 AM 5:00

PM Qualifications:

Active RN license (in good standing) for the state of practice At least one year of recent Home Health experience (field background strongly preferred) Demonstrated proficiency with Home Care Home Base (HCHB) electronic documentation OASIS assessment experience required Reliable transportation, valid drivers license, and current auto insurance

Key Responsibilities:

Conduct thorough OASIS assessments and develop individualized patient care plans Deliver skilled nursing services such as wound care, medication management, IV therapy, and chronic disease support Monitor patient conditions, assess progress, and communicate promptly with care teams and physicians regarding any changes Educate patients and their families on care processes, medication regimens, and preventive strategies Complete accurate, timely documentation in Home Care Home Base (HCHB) Collaborate closely with interdisciplinary team members (physical therapists, occupational therapists, social workers, and aides) Participate in case conferences, staff meetings, and ongoing education as part of professional development Comply with all professional, regulatory, and agency standards in care delivery and documentation
