Current Colorado Registered Nurses (R.N.) License, Cardiopulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred. Must have a valid driver's license, proof of insurance and have means of transportation. What You'll Be Responsible For In coordination with the Care Manager and other nursing staff, develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Care/Interdisciplinary Team Conducts a comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Care/Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conducts nursing re-assessment semi-annually and as needed. Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observes, records, and reports participant's condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants. About Us Senior Community Care of Colorado PACE (Montrose) provides healthcare and supportive services for individuals age 55 and older, helping them live safely, comfortably, and independently in their own homes and communities. As a Program of All-Inclusive Care for the Elderly (PACE) provider and the first of its kind in Western Colorado, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant's needs.
