Job description PDN Nurse A Physicians Home Care (APHC) is a locally owned business in Colorado Springs that was started by a family of a Special Needs child who is now an adult. We were founded in 2017 in hopes to be able to provide the best possible care to those who need in-home assistance. Our motto is where everyone is treated as family and our main goals are to give back and help others. We have had the pleasure of helping over 900 families and are licensed by the state of Colorado to provide In Home Support Services (IHSS) as well as Accredited by the state and ACHC to provide CNA, Skilled Nursing, and Therapy Services. Pay:

$37.00 per hour

Sign-On Bonus:

$2,500. Please inquire within for details.

Schedule:

10-hour Overnight- Sunday, Monday & Tuesday (days may be flexible) Schedule will be from 9:00pm- 7:00am, with flexibility.

Responsibilities of the LPN/RN role include: Assist as assigned with activities of daily living such as bathing, grooming, toileting and elimination, and adequate nutritional intake. Assist as assigned with ambulation, transfers and/or range of motion exercises. Furnish services, treatments and diagnostic and preventive procedures as assigned per the plan of care, following agency policies. Assuring the development, implementation, and updates of the individualized plan of care Initiate preventive and rehabilitative nursing procedures. Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition. Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Maintain and complete records of observations, treatments, and care of patient. Help update the patient plan of care. Participate in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech language pathologist. Accept and perform orders; LPN orders are cosigned by RN. Administer medications and treatments as ordered. Provide appropriate information about patient to other agencies/ individuals involved in patient's care as needed. Immediately report to RN Supervisor any patient incidents/variances or complaints and submit any changes in schedule daily

Other Job Requirements:
