  • Rewarding your expertise and commitment Join a team that is not only transforming healthcare but also prioritizing the people who make it possible What You'll Do Provide direct patient care, including assessments and care plan development Manage a caseload and coordinate care across the clinical team Administer medications and treatments per physician orders Monitor patient progress and adjust care plans as needed Educate patients and caregivers on care and self-management Communicate with physicians and care teams to ensure continuity of care Supervise and support HHAs and LPNs Complete accurate and timely documentation (OASIS, visit notes, orders) Ensure compliance with policies, regulations, and HIPAA standards What You Bring Active RN license Graduate of an accredited nursing program (BSN preferred) Home health or related behavioral health clinical experience Strong communication, organization, and critical thinking skills Current CPR certification Valid driver's license and reliable transportation
Physical Demands & Environment Mobility:
Frequent standing, walking, sitting, climbing, balancing, stooping, and kneeling.
Dexterity:
Continuous use of hands and fingers for fine motor tasks and equipment operation.
Lifting/Transfer:
Occasional lifting or pushing of loads (both under and over 25 lbs); includes occasional patient transfers.
Hazard Exposure:
Frequent exposure to bloodborne/airborne pathogens and infectious materials.
