What You'll Do:

Effectively screens and evaluates patients with varying clinical needs and develops appropriate care plans following both state and federal regulatory standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment intervention to patients along with improving the overall quality of life for those residents who are with us long term. Develop and support innovative therapy programs, pushing for new ways to deliver top quality care to our vulnerable population. Assesses patient's progress and adjust treatment plans accordingly.

What You'll Need:
