Requirements:

A current, active RN or LPN license in Connecticut Valid CPR certification Driver's license and reliable transportation Strong clinical judgment, communication, organizational, and time-management skills Comfortable with technology and electronic health records Previous home health experience preferred but not required Familiarity with Medicaid, Medicare, and Sandata EVV is a plus Bilingual Spanish-speaking clinicians encouraged to apply

Key Responsibilities:

(Depending on licensure) Perform complete patient assessments and develop individualized, evidence-based care plans Reassess and update care plans as patient needs change Provide hands-on nursing care, administer medications safely, and educate patients and family members Coordinate care with interdisciplinary team members and community resources
