65 Commercial Blvd Torrington (10413) Description - External The OASIS Registered Nurse (RN) provides skilled nursing care to patients in their homes and community settings, ensuring high-quality, outcome-based, and cost-effective care. This role specializes in completing comprehensive OASIS assessments, developing individualized care plans, and collaborating with interdisciplinary teams to support positive patient outcomes. Key Responsibilities Complete comprehensive patient assessments, including accurate OASIS clinical and functional evaluations. Review referrals and initiate patient and family contact. Develop and implement individualized plans of care in collaboration with patients, families, physicians, and care team members. Perform OASIS assessments throughout the patient's episode of care to ensure consistency and quality outcomes. Provide skilled nursing visits, assess patient progress, and evaluate responses to treatment. Educate patients and caregivers on disease management, treatment plans, and health promotion. Coordinate care with physicians, therapists, and other healthcare professionals. Supervise LPNs and Home Care Aides in carrying out the established plan of care. Maintain accurate and timely clinical documentation and reporting requirements. Participate in care conferences, interdisciplinary team meetings, SBAR reports, and quality improvement initiatives. Qualifications - External Required Graduate of an Associate Degree or Diploma Nursing Program. Current Registered Nurse (RN) license in the State of Connecticut. Minimum of one year of nursing experience or one year in a specialty nursing area. Strong computer skills. Preferred Bachelor of Science in Nursing (BSN) from an NLN-approved program. Medical-surgical and/or home health experience. Qualifications Required Graduate of an Associate Degree or Diploma Nursing Program. Current Registered Nurse (RN) license in the State of Connecticut. Minimum of one year of nursing experience or one year in a specialty nursing area. Strong computer skills. Preferred Bachelor of Science in Nursing (BSN) from an NLN-approved program. Medical-surgical and/or home health experience.
