The RN's overall responsibility is to provide direct patient care and case management to an assigned caseload, in the home setting or in facilities, in accordance with physician's orders. This includes admission of patients for service and determining and developing a plan of care. This position will cover the town of Ridgefield, CT and surrounding local area.
Job Responsibilities:
Provides direct patient care and case management. Competently implements 24-hour responsibility for each patient including nursing and home health aide service. Identifies and documents patient and family needs through comprehensive assessment. Reports all findings to M.D., coordinates services, and follows through on Physician orders. Performs admission procedure as per RVNAhealth policies using legible, accurate documentation. Develops a plan of care consistent with the physician's plan of treatment. Performs evaluation of patients' progress, promptly acts on any changes made to patient's plan of treatment or condition, including appropriate discharge procedure when goals have been met. Updates and maintains the nursing care plan. Documents communication and interprets plan of care to the patient regarding diagnosis and treatment. Qualifications/Expectations Graduation from an accredited School of Nursing. Currently licensed to practice as an RN in the State of Connecticut or Compact License required. Has a minimum of one year of nursing experience. Ability to drive/travel throughout the assigned territory. Why Join RVNAhealth? Compensation:
Our target pay range for this position is $95,000 to $103,000 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
