Care At Home - Nurse Practitioner - Litchfield County, CT Optum 401(k) United States, Connecticut, Watertown Aug 03, 2026 Litchfield & Waterbury, CT Optum Home & Community Care is seeking a Nurse Practitioner to provide in-home care for high-risk patients in Litchfield County, Connecticut. This role focuses on delivering comprehensive, longitudinal care in the patient's home while collaborating with an interdisciplinary care team to improve outcomes and reduce barriers to care. Position Highlights $40,000 Student Loan Repayment or $20,000 Sign-On Bonus
No nights, weekends, or holidays
Daily travel within the assigned territory
Provide in-home assessments and ongoing care management
Manage acute and chronic conditions, including post-acute rehabilitation needs
Conduct advanced care planning and patient education
Collaborate with interdisciplinary care teams
