JOB SUMMARY
Receives and processes referral intake information Demonstrates the ability to obtain and track Face to Face (F2F) documentation for applicable referrals Provides exemplary customer service to all referral sources Adheres to HIPAA Policies and Procedures Performs all other duties that Management may determine necessary Reports to Clinical Manager
QUALIFICATIONS
Minimum Education and Experience LPN license in good standing in the State of Connecticut One year experience as an LPN in an acute/sub-acute or home care setting Proficiency in the use of computers and software applications
Other Qualifications:
Physical, mental and sensory abilities sufficient to meet performance standards with or without assisted devices Effective oral and written communication skills Must have good organizational skills and be detail focused Ability to work in a fast paced environment with demonstrated ability to handle multiple competing tasks and demands
