Job Description

Interim HealthCare of Delaware is seeking a compassionate, detail-oriented, and experienced Full-Time Home Health Admissions Registered Nurse (RN) to join our clinical team. The RN evaluates patient progress, identifies changes in condition, updates individualized plans of care, and ensures accurate and timely OASIS and clinical documentation. The RN works closely with patients, families, physicians, and the interdisciplinary care team to promote continuity of care, support positive patient outcomes, and ensure services remain appropriate to each patient's individual needs. Essential Responsibilities Conduct comprehensive in-home Oasis admissions, continuations of care, resumptions of care, and recertification assessments to evaluate patients' physical, functional, emotional, and environmental needs. Complete OASIS assessments accurately and within required regulatory and company timelines. Evaluate patient progress toward established goals and determine the need for continued skilled services. Identify changes in patient condition, functional status, medications, caregiver support, and home environment. Develop, update, and revise individualized plans of care based on current assessment findings. Communicate patient status, clinical findings, and recommendations to physicians and members of the interdisciplinary care team. Provide skilled nursing interventions when indicated, including wound care, medication management, chronic disease management, catheter care, post-operative care, and patient education. Monitor patients for changes in condition and identify opportunities to prevent avoidable hospitalizations and emergency department visits. Complete accurate, timely, and thorough clinical documentation. Maintain compliance with Medicare and Medicaid, state, and company documentation and regulatory standards. Promote patient safety, independence, dignity, and overall quality of life. Qualifications Current Registered Nurse (RN) license in the State of Delaware. Minimum of one (1) year of recent nursing experience preferred. Previous home health experience strongly preferred. Previous OASIS and/or recertification experience preferred. Current CPR certification. Valid driver's license, reliable transportation, and current auto insurance. Ability to travel throughout assigned service areas. Strong clinical assessment, critical thinking, communication, and documentation skills. Excellent organizational and time-management abilities. Ability to work independently while effectively managing time, priorities, and patient needs. Successfully pass required background and abuse registry checks. Preferred Qualifications Previous Home Health experience. Previous OASIS assessment experience. Experience completing recertification assessments. Knowledge of Medicare Home Health Regulations and Conditions of Participation. Experience with electronic medical record (EMR) systems. Experience developing and updating individualized plans of care. Strong understanding of chronic disease management and skilled home health services.
