Job Description

CenterWell Home Health seeks a compassionate Clinical Care Nurse to deliver skilled in-home nursing services for diverse adult and senior patients. In this role, you will conduct comprehensive assessments, develop and implement individualized care plans, administer medications and treatments, and coordinate with physicians, therapists, and caregivers to optimize outcomes. You will educate patients and families on disease management, safety, and medication adherence while maintaining accurate clinical documentation. Responsibilities Perform comprehensive in-home nursing assessments for adult and senior patients Develop, implement, and regularly update individualized care plans Administer medications, treatments, and wound care per provider orders Monitor patient status, document clinical findings, and report changes promptly Educate patients and caregivers on disease management, safety, and medications Coordinate care with physicians, therapists, social workers, and other team members Ensure compliance with clinical protocols, infection control, and safety standards Maintain accurate, timely, and compliant electronic health records Support transitions of care and help prevent avoidable hospital readmissions Contribute to quality improvement initiatives and best-practice adoption Required Skills Comprehensive patient assessment Care plan development and management IV therapy and medication administration Wound care and dressing changes Chronic disease management (CHF, COPD, diabetes) Home health documentation (e.g., OASIS) Care coordination and case management Patient and family education Infection control practices Use of EMR/home health software
