Job Description

Trustbridge Hospice , a member of Empath Health, is currently seeking a caring and motivated RN Hospice Case Manager to join our team in the Boca area . If you are passionate about patient-centered care and want to make a profound impact in the lives of patients and families, this is your opportunity. You would work the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement (One weekend every 6 weeks) Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness. What you'll Do As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey.
You will:
Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group) Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust Why Join Empath Health?

Current Basic Life Support Certification. One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred. Understanding of and ability to apply characteristics of
Hospice Nursing:
case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs Basic computer skills and EMR software experience a plus. Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license. What You'll Find at
