We specialize in senior care, including Alzheimer's and Dementia care, committed to providing compassionate and respectful in-home services to our clients. Job Overview We are seeking a dedicated and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team. The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and a passion for delivering high-quality home health care. This role involves providing comprehensive nursing services, coordinating patient care plans, and ensuring compliance with healthcare standards within a home setting. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews. Administer medications, injections, and treatments such as IV infusions and catheter care with precision and adherence to aseptic techniques. Develop and implement individualized care plans tailored to each patient's needs, including memory care, hospice & palliative medicine, and behavioral health support. Monitor patient progress and modify care strategies accordingly while maintaining detailed medical documentation in electronic health records (EHR) systems. Collaborate with physicians and other healthcare professionals on diagnostic evaluations, medication management, and discharge planning. Provide education to patients and families on disease management, medication administration, nutrition, and safety protocols. Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information. Qualifications Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), with valid state certification. Experience working with individuals with disabilities, developmental delays, or in geriatrics is highly desirable. Proficiency in EMR/EHR systems such as Epic or Cerner; familiarity with
ICD-9/ICD-10
coding is preferred. Strong clinical skills including wound care (suturing), ostomy care, tube feeding, ventilator management, and laboratory specimen collection. Knowledge of infection control practices and aseptic techniques essential for safe patient care. Critical care experience in settings such as ICU or trauma centers is advantageous; experience in hospital medicine or emergency medicine is a plus. Excellent communication skills coupled with the ability to work independently within a team environment. Additional skills such as cardiology procedures (cardiac catheterization), dialysis experience, or experience working in long-term or assisted living facilities are beneficial.
