Registered Nurse (RN) - Home Health Baylor Full-Time | Friday-Monday Schedule Preferred Care Home Health is seeking an experienced Home Health Registered Nurse (RN) to join our growing clinical team in a Full-Time Baylor position working Friday through Monday . This role is ideal for an experienced home health RN who is comfortable managing a patient caseload, completing comprehensive assessments, documenting in OASIS, and providing high-quality skilled nursing care in the home setting. Position Responsibilities Provide skilled nursing care to patients in their homes. Manage an assigned home health patient caseload. Perform comprehensive nursing assessments and evaluate patient needs. Develop, implement, and update individualized plans of care. Complete OASIS assessments and required clinical documentation accurately and timely. Monitor patient condition and communicate changes in status to physicians and other members of the care team. Administer medications and provide skilled nursing treatments as ordered. Educate patients and caregivers regarding medications, treatments, disease processes, and home safety. Coordinate care with physicians, therapists, aides, social workers, and other members of the interdisciplinary team. Maintain compliance with Medicare, CMS, and home health regulatory requirements. Promote quality outcomes and positive patient experiences. Qualifications Current and unrestricted Florida Registered Nurse (RN) license . Minimum of 1 year of Medicare Home Health experience required. Minimum of 1 year of OASIS documentation experience required. Current CPR/BLS certification. Valid driver's license, reliable transportation, and current auto insurance. Strong clinical assessment, communication, organizational, and time-management skills. Ability to work independently and effectively manage a home health caseload. Knowledge of Medicare home health regulations and documentation requirements preferred.
