Philadelphia Home Health Care is entering an exciting new chapter under new leadership and a dedicated new Clinical Manager specifically brought on to support our field clinicians. We are seeking a reliable and dedicated Registered Nurse (RN) to serve our patients in the Brevard county area including Cocoa, Melbourne, Rockledge, Titusville, and Palm Bay . This role is perfect for a clinician who values autonomy, seeks a supportive "nurse-first" culture, and is committed to providing high-quality care. Registered Nurse (RN) The Opportunity As a Registered Nurse with Philadelphia Home Health Care, you will play a pivotal role in our mission to provide elite patient care across Central Florida. With our recent leadership transition, we are focused on streamlining communication and providing our RNs with direct access to clinical leadership to ensure you feel confident and supported in the field.

Key Responsibilities Comprehensive Assessment:

Conduct thorough physical and psychosocial assessments of patient needs in their home or facility environment.

Care Planning:

Develop, implement, and evolve individualized care plans in coordination with a multidisciplinary team.

Clinical Intervention:

Deliver high-quality nursing interventions, including wound care, medication management, and patient education.

Collaboration:

Act as a key point of contact between patients, families, and our new leadership team to ensure seamless care delivery.

Documentation:

Maintain accurate, timely clinical records in compliance with state and federal regulations. Qualifications Active, unencumbered RN License in the state of Florida. Proven commitment to high-quality patient care and clinical excellence.

Availability:

Flexible schedule, with a strong preference for candidates available at least 3 days per week (part-time hours). Strong communication skills and the ability to work effectively within a multidisciplinary team. Self-motivated with excellent time-management skills (ideal for those making their own schedules).
