In this vital role, you will provide compassionate, high-quality nursing care across the skilled nursing/ long-term care environment. Your proactive approach and clinical skills will ensure patients receive personalized care that promotes healing, comfort, and well-being. This position offers a dynamic work environment where your nursing knowledge and critical thinking skills are valued and essential to delivering exceptional patient outcomes. Responsibilities Perform patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems. Administer medications, injections, and treatments following strict protocols to ensure safety and efficacy. Monitor patient conditions closely, utilizing skills in airway management, ventilator care, tube feeding, and acute pain management to respond swiftly to changes. Coordinate with interdisciplinary teams for discharge planning, case management, and diagnostic evaluation to optimize patient recovery and long-term care plans. Maintain accurate medical records and documentation in compliance with HIPAA regulations and long-term care standards. Provide patient education on medication administration, wound care, dementia or Alzheimer's care, and health coaching tailored to individual needs. Support infection control practices and aseptic techniques during procedures such as venipuncture, suturing, and specimen collection. Qualifications Must have a valid RN or LPN license with current state licensure; additional certifications such as Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS). Knowledge of long-term care regulations and experience in skilled nursing facilities, preferred. Excellent communication skills for caregiver communication and patient/client interaction; ability to work effectively within multidisciplinary teams.
