- Dunedin, FL A leading healthcare organization is seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting.
This full-time opportunity offers a flexible schedule, professional development opportunities, and the ability to make a meaningful impact in the lives of patients throughout Pasco County. Responsibilities Provide hands-on nursing care to patients in their homes using the nursing process and established clinical standards and protocols Assess, plan, implement, and evaluate individualized patient care plans Oversee and coordinate the activities of the multidisciplinary team involved in patient care Monitor the delivery of therapeutic services through effective written and verbal communication with all disciplines Ensure all scheduled home visits are authorized and supported by physician orders Conduct weekly reviews of patient visits and communicate care plans and anticipated needs to the Patient Care Supervisor and team assistant Communicate schedule changes, caseload updates, and patient care concerns to the Home Care Supervisor Maintain accurate and timely documentation of patient assessments and care provided Educate patients and caregivers to promote health, independence, and positive outcomes Collaborate with interdisciplinary team members to ensure continuity and quality of care Qualifications Graduate of an accredited School of Nursing Current Registered Nurse (RN) license in the state of practice Current Basic Life Support (BLS) certification Valid Florida driver's license or ability to obtain one within 30 days of hire Associate Degree or Diploma in Nursing required Bachelor of Science in Nursing (BSN) preferred Minimum of one year of Registered Nurse experience required Previous home health experience preferred Strong communication, organizational, and critical thinking skills Ability to work independently and effectively manage patient caseloads Must meet all applicable background screening requirements Benefits Competitive compensation Medical, dental, and vision insurance Paid time off Retirement savings plan with employer contributions Tuition reimbursement and professional development opportunities Performance-based bonuses Employee wellness and discount programs Career growth and advancement opportunities Flexible scheduling and work-life balance How to Apply Qualified candidates are encouraged to submit their resume to: Please complete the online application to be considered.
