Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

JOB DETAILS POSITION SUMMARY
•RN Case Manager
POSITION DUTIES
•Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.
MINIMUM REQUIRED QUALIFICATIONS
•FL/compact RN license BLS 2 years case manager experience in an acute hospital setting Acute hospital experience in specialty during the past year with no more than a 90-day break Knowledgeable in Meditech, Midas, and Interqual.

•13 weeks
SHIFT / HOURS PER WEEK
•8:00AM-4:30PM with every other weekend SYSTEMS
•Meditech, Midas, Interqual
