Job Description

HCA Healthcare is seeking an RN Discharge (PRN) to support safe, efficient transitions of care at our Nashville, TN facility. In this role, you will assess discharge readiness, coordinate with physicians and case managers, and provide thorough patient and family education on medications, follow-up care, and home instructions. You will ensure accurate documentation, address discharge barriers, and contribute to readmission reduction and quality initiatives within a collaborative, patient-centered environment that supports learning and professional growth. Responsibilities Coordinate and manage safe, timely patient discharges from the hospital Conduct nursing assessments to confirm discharge readiness and patient stability Provide discharge education on medications, wound care, follow-up appointments, and self-care Collaborate with physicians, case managers, and interdisciplinary teams on discharge plans Ensure accurate, complete documentation of discharge plans and patient teaching Verify orders, prescriptions, and equipment needs prior to discharge Identify and address barriers to discharge, escalating complex cases as needed Reinforce patient safety instructions and symptom monitoring post-discharge Support quality, compliance, and readmission reduction initiatives Deliver patient-centered, compassionate care aligned with HCA standards Required Skills Patient discharge planning Acute care nursing Patient and family education Electronic Health Records (EHR) documentation Care coordination Clinical assessment Medication reconciliation Interdisciplinary team collaboration Regulatory and compliance standards Readmission reduction strategies
