HCA Healthcare is seeking an RN Discharge (PRN) to support safe, efficient transitions of care at our Nashville, TN facility. In this role, you will assess discharge readiness, coordinate with physicians and case managers, and provide thorough patient and family education on medications, follow-up care, and home instructions. You will ensure accurate documentation, address discharge barriers, and contribute to readmission reduction and quality initiatives within a collaborative, patient-centered environment that supports learning and professional growth.
