Qualifications & Experience:

Current, valid RN license in the state of Florida (or compact license) 2+ years of recent experience in RN case management or as a Case Manager In-depth understanding of discharge planning, utilization review, and care coordination Exceptional communication and interpersonal skills Strong analytical and problem-solving abilities Ability to collaborate effectively within multidisciplinary teams Familiarity with electronic health record (EHR) systems

Key Responsibilities:

Assess patient needs and develop individualized case management plans Coordinate care transitions, discharge planning, and follow-up services Advocate for patients while ensuring appropriate resource utilization Serve as a liaison between patients, families, physicians, and interdisciplinary teams Identify and resolve barriers to care to promote cost-effective outcomes Document all case activities and communications accurately in EHR systems Conduct ongoing evaluations of patient progress and update care plans as needed
