Job Description
HCA Healthcare is seeking a Registered Nurse Case Manager PRN to coordinate patient care and transitions across our Nashville facility. In this PRN role, you will perform comprehensive assessments, develop and manage individualized care plans, and collaborate with an interdisciplinary team to ensure safe, effective, and cost-conscious care. You'll educate patients and families, connect them with community resources, and support discharge planning.
