Home Health RN/LPN - Marathon, FLOverview S & V Family Home Health Agency is seeking a compassionate and dependable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide skilled home health nursing services for an adult patient located in Marathon, FL (33050) . The patient is being discharged home following hospitalization and requires skilled nursing for medication management, vital-sign monitoring, Foley catheter care, patient education, and ongoing assessment . The patient has a recent history of right upper-extremity DVT, atrial fibrillation, anemia, thrombocytopenia, urinary retention, and increased fall risk . The patient is also receiving anticoagulation therapy and requires careful monitoring for bleeding and other complications. Responsibilities Perform skilled nursing assessments and monitor the patient's overall condition. Obtain and document vital signs. Complete medication reconciliation and provide medication management and education. Monitor anticoagulation therapy and educate the patient regarding bleeding precautions and signs/symptoms that require medical attention. Provide Foley catheter care and management , including monitoring urine output and assessing for signs and symptoms of infection or urinary complications. Provide patient education regarding catheter care and urinary retention. Perform Foley catheter removal when ordered by the physician , followed by appropriate assessment and education regarding signs and symptoms of acute urinary retention. Assess cardiovascular, respiratory, neurological, gastrointestinal, genitourinary, skin, and functional status as clinically indicated. Monitor for complications related to the patient's recent DVT and hospitalization. Reinforce fall-prevention and home-safety education. Communicate changes in condition promptly to the physician and S & V Family Home Health Agency. Coordinate care with PT, OT, Home Health Aide, physicians, and other members of the patient's healthcare team. Complete timely and accurate home-health documentation in accordance with the plan of care and agency policies. Follow infection-control, HIPAA, and patient-safety requirements. Qualifications Current and unrestricted Florida RN or LPN license . Current CPR/BLS certification. Previous home health or skilled nursing experience preferred . Must be comfortable with Foley catheter care and removal . Experience caring for patients receiving anticoagulation therapy is preferred. Strong assessment, medication-management, and patient-education skills. Experience caring for adult/geriatric patients with multiple medical conditions and fall risk. Reliable transportation and ability to travel to the patient's home in Marathon, FL . Must meet all S & V Family Home Health Agency employment/credentialing requirements. Ideal Candidate We are looking for a nurse who is dependable, clinically confident, compassionate, and comfortable independently managing a medically complex patient in the home setting. The nurse should have strong assessment skills and communicate promptly with the agency and physician regarding any significant changes in the patient's condition.
