RN Crisis Care Per Diem

We are seeking a compassionate and driven Registered Nurse (RN) to join our Continuous Care team—providing one-on-one support when patients and families need it most. in the Broward County area while providing exceptional care on the following schedule: Per Diem 2 days 7am - 7:30pm and weekend availability. What you'll Do As a Continuous Care RN, you'll be at the heart of meaningful care—bringing comfort, expertise, and reassurance directly to patients and families. Deliver exceptional, compassionate care aligned with our mission and values Assess and identify patient and family needs with empathy and clinical expertise Coordinate individualized care under the direction of the primary physician Collaborate as a key member of the Interdisciplinary Group (IDG) Implement and support personalized plans of care focused on comfort and quality of life Utilize monitoring, intervention, and teaching skills to enhance patient well-being Provide reassurance, education, and emotional support to families Ensure a consistently high level of personalized, engaging service Address concerns promptly to ensure complete patient and family satisfaction Uphold the highest standards of confidentiality and respect What You'll Need Registered Nurse currently licensed in the State of Florida. BSN preferred Hospice certification preferred. Current Basic Life Support Certification. One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred. Understanding of and ability to apply characteristics of

Hospice Nursing:

case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs Basic computer skills and EMR software experience a plus. Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license. Why Join Empath Health?
