Full-Time Salaried Position Hours:
Mon-Friday 8am-5pm.
Location:
Sits in our Inpatient Unit with Hospice SFM and could round to Palm Coast and Palm Coast Parkway as needed for face to face visit and within the community. Evaluates the health status of selected patients, orders treatment plans and provides education to patients and their families under the supervision of a licensed physician. Performs clinical evaluations and reviews diagnostic studies of patients under the supervision of a licensed physician. Participates in interdisciplinary team conferences of the facility teams, as needed. Interacts with patients and families to manage the psychological, social and family issues regarding hospice and the services rendered. Collaborates and consults with the hospice management team to facilitate problem solving toward meeting patient needs. Completes all documentation timely and accurately in the patient records on daily visits. Participates in staff meetings, department meetings, in services and other related activities. Attends educational and developmental seminars and meetings, as deemed appropriate. Provides necessary billing documentation in a timely manner to the hospice Finance department. Performs all other duties as assigned
Education:
Master's of Nursing [Required]
Field of Study:
N/A Work Experience:
3+ years of experience in medical nursing, hospice and/or critical care [Required] 2+ years of experience as an APRN [Required]
