Monday Friday, 8:00 AM 5:00

PM Key Qualifications:

Active RN License (state of practice, in good standing) At least 1 year of recent Home Health nursing experience (field-based strongly preferred) Proficiency with Home Care Home Base (HCHB) electronic documentation system Experience completing OASIS assessments Reliable vehicle, current drivers license, and auto insurance

Responsibilities:

Perform comprehensive OASIS assessments and develop tailored care plans for each patient Deliver skilled nursing services, including wound care, medication management, IV therapy, and chronic disease management Monitor patient progress, recognize and respond to changes in condition, and communicate effectively with physicians and care team members Educate patients and families about disease progression, treatment, medication, and preventative care strategies Ensure timely, accurate documentation in Home Care Home Base (HCHB) Collaborate with multidisciplinary team members, including therapists, social workers, and aides, to promote coordinated care Attend case conferences, staff meetings, and engage in ongoing professional development Adhere to all regulatory and agency standards
