Position Summary We are seeking a compassionate and skilled Registered Nurse (RN) to provide in-home nursing care to patients across Palm Beach County. This role offers flexibility and independence, while allowing you to build meaningful relationships with patients and families. You will play a key role in assessing patient needs, coordinating care, and supporting individuals through recovery, chronic condition management, and transitions from hospital to home. What You'll Do Perform comprehensive in-home patient assessments (OASIS experience preferred) Administer medications and treatments as ordered by physicians Monitor patient conditions and respond to changes appropriately Educate patients and families on disease management and care plans Coordinate care with interdisciplinary teams to ensure optimal outcomes Complete timely and accurate documentation in compliance with regulations Support patients transitioning from hospitals, rehab, or skilled nursing facilities What You Bring Active Florida RN license Strong clinical assessment and critical thinking skills Experience in home health or similar setting preferred Familiarity with Medicare and Managed Care guidelines Proficiency in: Wound care & wound vacs IV therapy & PICC lines Blood draws PEG tubes & trach care Foley catheter care Diabetic management Excellent communication and patient education skills Why PRC?
