Job DescriptionJob Description Home Health RN $60,000•$80,000/year | Full-Time St. Mark Village | Palm Harbor, FL Join St. Mark Village in Palm Harbor, FL as our Home Health RN and provide one-on-one care through our on-campus home health program. Support residents' independence and quality of life while working alongside an experienced interdisciplinary team in a rewarding retirement community setting.
Schedule & Pay Highlights Reliable Schedule:
This is a full-time role, Monday•Friday 8:30am•5:00 pm
Competitive Pay:
$60,000•80,000/year + credit given for experience
Convenient Setting:
Services are provided within the St. Mark Village campus
Meaningful Work:
Provide clinical support and individualized care to residents within our home health program
Collaborative Team:
Work alongside an experienced interdisciplinary care team dedicated to exceptional resident outcomesWhat You'll DoComplete Start of Care assessments, admissions, recertifications, discharges, and skilled nursing visits for home health patientsPerform comprehensive patient assessments and coordinate care plans with physicians and the interdisciplinary teamComplete accurate OASIS documentation and maintain compliance with Medicare and home health regulationsMonitor patient progress and identify changes in condition that require intervention or follow-up careEducate residents and families regarding treatment plans, medications, and disease managementFollow all home health regulations, safety, infection control, documentation, and mandatory reporting policiesWhat You'll NeedCurrent Florida Registered Nurse License (required)Previous home health nursing experience (required)OASIS documentation experience and knowledge of home health regulations (required)Current CPR certification (required)Strong assessment, communication, and clinical documentation skillsKnowledge of Medicare regulations and ACHC standardsBenefits Available to YouMedicalDentalVisionHSAShort and Long-term DisabilityVoluntary Life & AD&D InsuranceEmployee Assistance Program Apply today to join the St. Mark Village Home Health team and provide exceptional care to residents through our on-campus home health program.
