Job Description
Overview We are seeking a compassionate and dedicated Home Health Nurse to provide quality nursing care to patients in their homes. This role involves delivering personalized healthcare services, monitoring patient progress, and collaborating with healthcare teams to ensure optimal patient outcomes. The ideal candidate will be flexible, reliable, and committed to improving the well-being of our patients in a home setting. The position requires availability for at least three days per week, including one weekend day, with options for 8-hour or 12-hour shifts. Start dates are flexible within 1-2 weeks, so please let us know your availability. Responsibilities Provide comprehensive nursing care to patients in their home environment, including assessments, medication administration, and wound care. Monitor patient health status and document observations accurately to inform ongoing treatment plans. Educate patients and their families on health management, medication adherence, and safety procedures. Coordinate with physicians and other healthcare professionals to ensure continuity of care. Manage your schedule to accommodate at least three workdays per week, including one weekend day. Adapt to available shift lengths, whether 8-hour or 12-hour shifts, based on patient needs and personal availability. Maintain compliance with all safety protocols and healthcare regulations during home visits. Experience Valid nursing license (RN or LPN) in good standing. Prior experience in home health nursing or community-based care is preferred but not mandatory; training will be provided as needed. Strong communication skills and the ability to work independently in a home setting. Flexibility to start within 1-2 weeks of offer acceptance and adapt to varying shift schedules. Availability for at least three days per week, including one weekend day, with the ability to work either 8-hour or 12-hour shifts.
